By 1983, American R&B singer Patti LaBelle was five albums into her moderately successful solo career, but had yet to deliver a chart-topping hit single. After the poor commercial performance of her first full-length for Philadelphia International Records, 1981's The Spirit's In It, LaBelle was teamed up with producers Dexter Wansel and Cynthia Biggs, who began working with the singer on her sixth studio album in 1982. Due to LaBelle's busy schedule, the songs took over a year to complete, and "If You Only Knew," the album's lead single, appeared in November 1983. A smooth mid-tempo ballad about a woman opening up to an unsuspecting love interest, the Wansel-Biggs song quickly climbed to the top of the R&B charts and peaked at Number 46 on the Billboard Hot 100, becoming the first Number 1 single of her solo career and also her first pop crossover hit. Thanks to this success, its parent album I'm In Love Again achieved Gold status in the United States and reached Number 4 on Billboard's Top R&B/Hip-Hop Albums chart. In time, the track became one of Patti LaBelle's signature songs and was included in the live albums Live! (1992) and One Night Only! (1998)
